2013 Norovirus Gastroenteritis Associated with a Restaurant, Maple Grove, Minnesota

In April 2013 public health investigators in Minnesota learned of gastrointestinal illnesses that occurred among persons who attended a March 31 Easter brunch at a restaurant located in Maple Grove, Minnesota. Minnesota Department of Health (MDH) and Hennepin County Human Services and Public Health Department (HSPHD) launched an outbreak investigation. Forty-six patrons were interviewed. Twelve people met the case definition. Three restaurant patrons tested positive for Norovirus GI. Cases reported eating a variety of foods served buffet style. By univariate analysis, mashed potatoes, beans, fruit salad, dessert, and fresh fruit were associated with illness. After multivariable logistic regression, only mashed potatoes remained associated with illness. No one was hospitalized and there were no deaths. Illness histories and job duty information were obtained from 48 restaurant employees. Eight employees reported having gastrointestinal symptoms in the 2 weeks prior to the brunch. Illness onsets among employees ranged from March 24 to March 31. Two employees also reported illness on April 2 and April 4. No stool specimens were obtained from employees. HSPHD Environmental Health staff noted many problems at the restaurant. They did not have a certified food manager. Several violations related to potential cross contamination were observed including lack of employee handwashing after touching raw protein and soiled food contact surfaces. Employees reported bare-hand contact and no glove use when handling ready-to-eat foods. Several violations related to time-temperature abuse were identified. There was no documentation of employee illness on the illness log, and employees with gastrointestinal illness were not excluded from work until 24 hours after recovery.
