Pork or Fresh Cilantro 2005

Between June and September, a cluster of Salmonella Manhattan isolates was noted by the Minnesota Department of Health Laboratory. Five of the six isolates were genetically matched using pulsed-field gel electrophoresis (PFGE), a genetic fingerprinting technique. The other isolate varied by two bands, a close match. The sudden occurrence of this very rare serotype in Minnesota prompted an investigation. Three cases occured in June, and another three cases occurred in August and September. None of the first cluster had any common exposures, but two of the three in the second cluster attended the same wake. This led investigators to focus on the food served at the wake, Vietnamese pork sandwiches. The sandwiches contained pork, fresh cilantro, chicken pâté, homemade (pasteurized) mayonnaise, carrots and marinated daikon, jalapeños and cucumbers. An inspection of the catering restaurant found no major violations. A case-control study using non-Manhattan serotype Salmonella cases from the same time period was performed. Eating pork or cilantro was associated with infection. Due to the small number of infected people, epidemiologists were unable to determine which food was the source of the infections.
